I give First Academy permission to make a thorough investigation of my entire work history and to verify all data given in my application for employment, related papers, and oral interviews. I release from liability any person giving or receiving any such information. I hereby certify that the answers and other information on this application are true and correct and that I understand any misrepresentation or omission of facts on my part will be justification for separation from First Academy if employed. I understand that my employment may be contingent upon receipt of alien registration numbers, verification of birth, and any other pertinent information bearing upon my employment. I also understand that my continued employment depends upon the will of First Academy/First Baptist Church or myself. Please note we reserve the right to run a Criminal Background Check on all applicants.
